Splenic Marginal Zone Lymphoma (SMZL) - Peripheral Blood Involvement: Bone marrow is involved in virtually all cases of SMZL, whereas peripheral blood involvement is seen in 55-65% of cases. A small number of neoplastic lymphocytes with villous morphology are commonly seen in peripheral blood smears.

The villi in SMZL are seen in only a subset of lymphocytes and consist of short cytoplasmic projections, often localized to one pole of the cell. In contrast, the villi in Hairy Cell Leukemia are longer and located circumferentially.
