Gross Pathology: Myelolipoma is a soft, well-circumscribed mass ranging in size from 3 to 8 cm but some may reach enormous dimensions (as large as 34 cm and weighing more than 6000 gm). The appearance on cut surface depends upon the relative proportions of mature fat and hematopoietic elements. When fat is the predominant component, it grossly has a lipoma-like appearance. When hematopoietic elements are abundant, it has a variegated appearance with gelatinous, reddish-brown areas admixed with fat.

This case was discovered incidentally in a 77 y/o female. It has a variegated appearance and is surrounded by a thin bright yellow rim of adrenal cortex. Image copyright: pathorama.ch
