Myelolipoma : Differential Diagnosis

Differential Diagnosis: The differential diagnosis of myelolipoma includes extramedullary hematopoietic tumor, lipid-rich adrenal cortical adenoma, lipoma, retroperitoneal liposarcoma, renal angiomyolipoma, and teratoma.

Extramedullary hematopoietic tumor is usually seen in response to severe anemia (associated with thalassemia, hereditary spherocytosis etc), myeloproliferative diseases, myelosclerosis, and skeletal disorders. It is multifocal and associated with hepatosplenomegaly.

The image shows an adrenal myelolipoma. There is a thin rim of adrenal cortex (zona fasciculata) near the top. Underneath are bone marrow elements with trilineage hematopoiesis and numerous mature fat cells.
