Microscopic Features of Gynecomastia: The ducts show a variable degree of epithelial hyperplasia and are surrounded by a cuff of hypocelluar myxomatous stroma that appears lighter in color. The edematous stroma contains large amounts of acid mucopolysaccharides. The ducts are present in a dense hyalinized stroma which may show features of pseudoangiomatous stromal hyperplasia. Squamous metaplasia may be seen in ductal epithelium. More recent lesions show greater degree of epithelial hyperplasia and even micropapillary architecture. Long-standing cases show more prominent fibrosis.
