When faced with a high-grade carcinoma in a prostate needle biopsy, the features favoring urothelial carcinoma include: tumor growing in nests; marked nuclear pleomorphism with giant tumor nuclei; dense eosinophilic cytoplasm; squamous differentiation; necrosis; positivity for p63 and HMWCK; negativity for prostatic markers.
