Comments:
Paget Disease of Bone (PDB) - Imaging Studies: Lumbar spine is one of the most commonly involved sites in PDB. The affected vertebrae initially show mixed lytic and sclerotic areas and in later stages a uniform increase in radiodensity. There may be loss of height as well as increase in the width of the vertebra, creating a picture frame appearance (better seen in the next image). The pelvic bones often show a combination of lytic areas and increased radiodensity. The disease bone has striated or honeycombed appearance.

Case History: This CT shows polyostotic Paget disease of bone in a 70 y/o male. There was involvement of several lower thoracic vertebra, L3, right hemipelvis, sacrum, right knee and left proximal femur. Note the coarse trabeculation in the affected areas (arrows). Case courtesy of Dr Alexandra Stanislavsky, Radiopaedia.org. From the case rID: 12871
