Discrete large transformed cells with morphology of centroblasts or immunoblasts are usually present in small numbers in thyroid MALT lymphomas. Several such cells can be seen in this image.

If they form solid aggregates, nodules or confluent sheets, a diagnosis of diffuse large B-cell lymphoma (DLBCL) should be considered. Less than 10% of MALT lymphomas transform to DLBCL. The DLBCL component may not completely overrun the background MALT lymphoma. In such cases, the presence or absence of MALT lymphoma and the relative proportion of both components should be mentioned in the report.
