Gastric MALT Lymphoma - Differential: Gastric MALT lymphomas may be difficult to differentiate from benign chronic inflammatory conditions. Features favoring lymphoma over benign gastritis include: glandular destruction, lymphoepithelial lesions, and dense lymphoid infiltrate in the muscularis mucosae.

This image shows an example of plasmacytoid variant of gastric MALT lymphoma. Vast majority of the cells have plasmacytoid appearance. Such cases are often CD20 negative but should be CD79a+ and show kappa or lambda restriction. The plasma cells are often atypical with binucleation and variation in size and shape.
