Differential Diagnosis: In infants, cardiac fibromas can be quite cellular and difficult to distinguish from low-grade fibrosarcomas. As a general rule, cardiac sarcomas are exceedingly rare in children. Any fibrous tumor arising in the heart in children is likely to be benign.

In adults, cardiac fibromas can microscopically resemble scar (healed infarct). However, there is no mass lesion and there is history of coronary artery disease. Pericardial solitary fibrous tumors may look similar to fibromas; however, they are attached to the pericardium and do not invade the myocardium. Besides, they are found in adults. Inflammatory pseudotumors can occur in heart and blood vessels; however, they are extremely rare. They have a prominent vascular and inflammatory component which is usually lacking in fibromas.
