Grading of mucoepidermoid carcinomas: The three-tiered grading system proposed by the AFIP is currently favored over the previous two-tiered system consisting of low- and high-grades. The AFIP system is based on points given for each histologic feature: intracystic component greater than 20%, 2 points; perineural invasion, 2 points; necrosis, 3 points; 4 or more mitotic figures/10 HPF, 3 points; anaplasia, 4 points. A score of 0-4 = low-grade tumor; score of 5-6 = intermediate-grade; and a score of 7 or more = high-grade tumor.
