Prognosis of Myxoid Liposarcoma: The local recurrence rate is 12-25%. Distant metastases occur in 30-60% of cases and may be delayed by several years. The risk of metastasis is directly related to the amount of high-grade (round cell) component. The common metastatic sites include other soft tissues as well as lungs and bones. About one-third of patients die of their disease.

Adverse prognostic indicators include: age < 45 years, the amount of high-grade cellular component (previously termed round cell), p53 overexpression, necrosis. This image shows high-grade (round cell) component as well as necrosis.

Given that the amount of round cell component has prognostic implications, it is important to quantify it and different ways have been proposed, including a two-tiered system (< 5% and > 5%) and a three-tiered system (0-5%, 6-25%, & > 25%). For accurate estimation, the tumor should be thoroughly sampled.
