LCAs related to androgenic steroid use may show cytologic abnormalities consisting of nuclear pleomorphism and atypia, multinucleation, prominent nucleoli, and/or an acinar (pseudoglandular) pattern. Activating beta-catenin gene mutations are present in 10-15% of LCA with cytologic atypia and acini and such cases are more likely to transform to hepatocellular carcinoma. Note the prominent cytoplasmic lipofuscin pigment, which is commonly seen in LCA.
