Lymph Node Involvement in Systemic Mastocytosis : Differential Diagnosis: The paracortical distribution of mast cell infiltrates, clear cytoplasm of mast cells, vascular proliferation, and eosinophilia may lead to mistaken diagnosis of peripheral T-cell lymphoma. When mast cells infiltrate lymphoid follicles, it can mimic follicular lymphoma.

The correct diagnosis can be arrived at with the help of a panel of immunostains consisting of CD117, CD25, CD2, and tryptase. Mast cells show positivity for most of these markers.
