Lymphomatoid granulomatosis is predominantly a pulmonary disease with bilateral lung involvement in >90% of cases at presentation.

This image shows polymorphous infiltrate filling up the alveolar spaces. The predominant cell population is reactive T-cells, which are admixed with scattered large atypical lymphoid cells, histiocytes, and a few plasma cells. There are no neutrophils, eosinophils, multinucleated giant cells or granulomas.
