Endometrial Hyperplasia (EH) - Differential Diagnosis: Secretory changes can show morphologic overlap with EH. The nuclei of secretory glands are enlarged and may show pleomorphism and loss of polarity. Frequently, there is glandular overcrowding. Features favoring secretory changes over EH include: predecidualized appearance of stroma, cytoplasmic vacuolization, and serrated glandular architecture at low power.

Atypical endometrial hyperplasia/endometrioid intraepithelial neoplasia may occur within a background of secretory endometrium.
