Differential Diagnosis of Thecoma (continued from previous image):

Adult Granulosa Cell Tumor (AGCT): The diffuse pattern of AGCT with luteinization or a prominent thecomatous component can mimic thecoma. However, AGCT will show areas of more typical granulosa cell differentiation as well as nuclear grooves elsewhere in the tumor. AGCT also lacks the pericellular fibrillar network with reticulin stain, unlike thecomas.

Sclerosing Stromal Tumor (SST): Both SST and thecomas can show spindle and epithelioid cells in a nodular growth pattern. Unlike thecomas, SST occurs in younger age group (under 30 years of age) and rarely produces hormonal manifestations. SST is more vascular and has hemangiopericytomatous areas.
