Metastatic melanoma can mimic adult GCT. Features favoring metastatic melanoma include melanin pigment, positivity for S-100 protein and HMB-45, negativity for alpha-inhibin and calretinin, and nuclear pleomorphism with prominent nucleoli.

Metastatic lobular breast carcinoma: The gyriform or watered-silk pattern of adult GCT can resemble metastatic breast carcinoma, especially the single-file (Indian file) pattern of invasive lobular carcinoma (as shown here). The diagnosis of breast cancer metastases is favored by history of breast cancer, bilaterality, lack of nuclear features (grooves), intracellular mucin, negative alpha-inhibin, positivity for EMA and GCDFP-15.
