Differential Diagnosis of Adult Granulosa Cell Tumor (GCT) of Ovary: continued from prev. image

Small Cell Carcinoma, Hypercalcemic Type (SCC, HT): Features favoring SCC, HT include advanced stage at presentation, young patient, no estrogenic manifestations, hypercalcemia (two-thirds of cases), high nuclear grade and immunohistochemical profile (positive for keratin, EMA; negative for alpha-inhibin and FOXL2).

High-grade Serous Carcinoma: Cystic adult GCT with pseudopapillary pattern can raise the concern for serous carcinomas. However, at higher magnification, adult GCT will not show high-grade nuclei, necrosis, or increased mitotic activity, unlike high-grade serous carcinoma.
