MICROSCOPIC FEATURES (continued from the previous image): Mature cystic teratomas can sometimes rupture and spill keratin and sebaceous material into the abdominal cavity, inciting a vigorous foreign body giant cell reaction. The resultant granulomatous peritonitis can mimic the appearance of metastatic carcinoma during exploratory laparotomy.

Melanosis peritonei: Rupture of a mature cystic teratoma containing adnexal structures with melanin can result in melanosis peritonei. Pigment-laden macrophages are present within the peritoneal stroma.

About this image: Mature cystic teratoma with a focus of blue nevus which was visible grossly as a dark purple-brown area. Sections showed clusters and sheets of melanin-containing cells beneath the squamous mucosa.
