Microscopic Features: The most common architectural pattern in adult granulosa cell tumor of the testis is diffuse sheets with no organized structure.

Less common but more characteristic is the microfollicular pattern, which shows numerous rosette-like structures resembling Call-Exner bodies of the Graafian follicle. The structures contain eosinophilic debris or hyalinized basement membrane material surrounded by a circular row of granulosa cells. Several Call-Exner bodies are seen in this image.

Other patterns include macrofollicles (resembling follicle cysts), trabeculae, nests, gyriform pattern as well as anastomosing cords.
