Both testis and epididymis appear congested. Epididymis is enlarged and shows pockets of fibrino-purulent exudate on its cut surface. The clinical course of acute epididymo-orchitis may be complicated by the formation of a scrotal pyocele, chronic draining scrotal sinus, and even testicular infarction.
