Transitional cell carcinomas of the ovary morphologically resemble malignant Brenner tumor; however, they lack benign and borderline Brenner elements. They differ clinically from malignant Brenner tumor, have worse prognosis, and are now classified as variants of high-grade serous ovarian carcinomas.

They show a variety of architectural patterns, including papillary, diffuse, insular, trabecular, and microcystic. They have high-grade pleomorphic nuclei, brisk mitotic activity, stromal invasion, and necrosis.
