Comments:
Lipoblastoma - Imaging: This MRI (Sagittal T1 - left panel; Axial T1 - right panel) image is from a 2 y/o male with a rapidly enlarging right paravertebral mass. It was well-circumscribed, measured 13 x 5.5 x 2.5 cm and had high signal on T1 and T2 with fat suppression on fat sat sequences - typical for fatty lesions. Note the internal septations in the axial image. The patient's age and the MRI findings are suggestive of a lipoblastoma. The main differential diagnosis is liposarcoma, which is extremely rare in children under 10 years of age.
