Comments:
Aneurysmal Bone Cyst (ABC) - Imaging: MRIs show multiloculated or a septated lesion with fluid-fluid levels created by the separation of serum and blood products within the cyst contents. The patient must remain still for at least 10 minutes for the blood components to settle within the cyst before imaging.

In about 50% of cases, the radiographic features suggest a benign process. In the remainder, the imaging findings are either indeterminate or suggest malignancy.

This MRI (coronal T2 fat sat) is from the same case as image 3 and shows expansile lesion of proximal right fibula that is T2 hyperintense with numerous fluid-fluid levels. Note the surrounding soft tissue inflammation.
