Splenic involvement in follicular lymphoma causes uniform enlargement of white pulp. Grossly, this appears as numerous white nodules throughout the splenic parenchyma. The patient was a 82 y/o female with follicular lymphoma involving infra- and supradiaphragmatic lymph nodes, spleen, liver, kidneys, bladder, epicardium and parietal pleura on autopsy.
