The three glands in the right half of the image show high-grade dysplasia. The glands on the left show low-grade dysplasia. High-grade dysplastic changes include nuclear hyperchromatism, irregularity in nuclear size and shape, nuclear stratification, prominent nucleoli, and increased mitotic activity. These changes can be seen in the glands on the right. The glands on the left have orderly architecture and only show prominent nucleoli.
