Comments:
This image shows spindle cells in intersecting fascicles admixed with gemistocyte-like cells, as well as a few highly pleomorphic astrocytes. Mitotic activity or necrosis are not seen, except in cases that have undergone malignant transformation (WHO Grade III)(seen in 15% to 20% of cases). Given the pleomorphism, the most common differential diagnostic considerations include glioblastoma, gliosarcoma, and pleomorphic sarcoma. Some cases of pleomorphic xanthoastrocytoma are admixed with ganglioglioma.
