Comments:
Osteosarcoma in Paget Disease of Bone (PDB): Imaging studies often show a poorly-demarcated, infiltrative, lytic lesion with or without mineralization involving the Pagetic bone. It is often accompanied by a pathologic fracture, erodes through the cortex and tends to form a bulky soft tissue mass. Bone scans show reduced isotope uptake in the lytic areas.

This plain X-ray shows osteosarcoma involving femur in the setting of PDB. It has aggressive, permeative pattern, sun burst like pattern, codman triangle, and wide zone of transition. Case courtesy of The Radswiki, Radiopaedia.org. From the case rID: 11727
