Comments:
Most cases of prostatic xanthoma can be correctly diagnosed on H&E features alone. In rare cases, the cytoplasmic features and the infiltrative pattern can create confusion with hypernephroid pattern of high-grade prostatic adenocarcinoma. Strong immunoreactivity of lesional cells for CD68 (shown here) and negativity for PSA and PAP will confirm the diagnosis of xanthoma.
