Multilayered epithelium (MLE) has been proposed as the potential precursor of intestinal metaplasia in Barrett esophagus. MLE shows columnar epithelium at the top over a layer of squamous cells at the basal portion. MLE is frequently seen in patients with gastroesophageal reflux disease. This image was taken from an esophagectomy specimen from a patient with adenocarcinoma arising in Barrett esophagus. The focus was adjacent to intestinal metaplasia.
